The Surgical Treatment of Bile Duct Stones


Abstract
The clinical observation and review of 102 cases who had been made by-pass procedure and partial hepatectomy for the bile duct stones at Department of Surgery, Dae Jeon Eul Ji General hospital from September, 1981 to December, 1991 was performed.
@ES The results were obtained as follows:
@EN 1) Operative procedures were as follows; choledochoduodenostomy; 67 cases(65.7%), Roux-en-Y choledochojejunostomy; 10 cases(9.8%), sphincteroplasty; 7 cases(6.9%), hepaticojejunostomy; 1 cases(1%), partial hepatectomy; 17 cases(16.6%).
2) There indications for by-pass surgery of common bile duct and partial hepatectomy were as follows; intrahepatic stones; 61 cases(59.8%), primary common bile duct stones; 30 cases(29.4%), residual or recurred stones; 11 cases(10.8%).
3) The location of intrahepatic stones(67 cases) were as follows; both; 36 cases(53.7%), left; 25 cases(37.3%) right; 6 cases(9%).
4) Operative mortality was 2.0%(2 cases), one for hemobilia after choledochoduodenostomy and the second for fulminant hepaitis after left lateral segmentectomy of liver.
5) The incidence of residual stones were 14.5%(1 cases) in by-pass procedure of common bile duct and 23.5%(4 cases) in partial hepatectomy.
6) Of the 82 follow-up cases, there were 7 recurred stones(by-pass procedure of common bile duct; 6 cases, partial hepatectomy; 1 cases), 5 cholangitis(by-pass procedure of common bile duct; 4 cases, partial hepatectomy; 1 cases), and one
intrahepatic
abscess in partial hepatectomy.
